PARTICIPANT INFORMATION

1. ACKNOWLEDGMENT OF RISK

I understand and acknowledge that participation in martial arts, combat sports, fitness training and related activities at Reizen MMA LLC (“Reizen MMA”) involves inherent and significant risks of injury. These activities include, but are not limited to:

• Brazilian Jiu-Jitsu and submission grappling

• Muay Thai

• Mixed Martial Arts (MMA)

• Wrestling

• Boxing and other striking activities

• Sparring and live training

• Throws, takedowns, sweeps and grappling

• Chokes, joint locks and submission techniques

• Pad work and heavy-bag training

• Strength, conditioning and cardiovascular exercise

• Use of mats, bags, training equipment and other gym facilities

• Training with coaches, members, students, guests and other participants

I understand that these activities involve physical contact and may result in injuries including, but not limited to, bruises, cuts, sprains, strains, dislocations, fractures, dental injuries, eye injuries, concussions, traumatic brain injuries, spinal injuries, paralysis, illness, permanent disability, and death.

I understand that injuries may occur even when rules are followed, protective equipment is used, and reasonable precautions are taken.

2. VOLUNTARY ASSUMPTION OF RISK

I voluntarily choose to participate in activities at Reizen MMA with knowledge of the risks involved.

I knowingly and voluntarily assume the inherent risks associated with my participation, including risks arising from physical contact with other participants, training surfaces, equipment, falls, throws, strikes, submissions, takedowns and strenuous physical activity.

I understand that I am never required to participate in sparring, live rolling or any particular exercise and may stop participating at any time.

3. RELEASE AND WAIVER OF LIABILITY

To the fullest extent permitted by Illinois law, I release and discharge Reizen MMA LLC, together with its owners, members, managers, instructors, coaches, employees, contractors, volunteers, agents and representatives (collectively, the “Released Parties”), from claims arising from the ordinary risks of my voluntary participation in activities at Reizen MMA, including claims based upon ordinary negligence to the extent such claims may lawfully be waived.

This release is not intended to waive liability that cannot legally be waived under applicable law, including liability for willful or wanton misconduct where such liability cannot lawfully be released.

4. PARTICIPANT RESPONSIBILITY

I agree to:

• Follow the instructions of Reizen MMA coaches and staff.

• Follow all gym safety rules and policies.

• Use appropriate protective equipment when required.

• Immediately stop training and inform a coach if I believe I am injured or unable to continue safely.

• Respect a training partner’s verbal submission or physical “tap” immediately.

• Exercise reasonable control during sparring and live training.

• Not intentionally injure another participant.

• Not participate while impaired by alcohol, illegal drugs or any substance that makes participation unsafe.

I understand that failure to follow safety rules or coach instructions may result in removal from training or termination of membership.

5. HEALTH AND FITNESS TO PARTICIPATE

I represent that I am physically capable of participating in the activities I choose to undertake.

I understand that Reizen MMA and its coaches are not medical professionals and are not responsible for determining whether I am medically fit to participate.

I agree that it is my responsibility to consult an appropriate medical professional if I have questions or concerns regarding my ability to participate safely.

I agree to notify a coach of any condition or injury that may materially affect my ability to participate safely when appropriate.

6. COMMUNICABLE CONDITIONS

I agree not to participate in training when I knowingly have a contagious skin condition, infection or other communicable illness that could reasonably place other participants at risk.

I understand that combat sports involve close physical contact and that exposure to communicable illnesses or skin conditions is an inherent possibility despite cleaning and sanitation procedures.

7. EMERGENCY MEDICAL CARE

If I am injured or become ill while participating and I am unable to make decisions for myself, I authorize Reizen MMA personnel to contact emergency medical services and obtain reasonably necessary emergency assistance.

I understand that Reizen MMA does not guarantee the availability of medical personnel at the facility.

I understand that I am responsible for medical expenses incurred on my behalf, subject to applicable law and any rights I may have against another person or entity.

8. PERSONAL PROPERTY

I understand that Reizen MMA is not responsible for lost, stolen or damaged personal property except to the extent responsibility cannot legally be disclaimed.

I am responsible for securing my belongings while at the facility.

9. PHOTO AND VIDEO RELEASE Please select one:

I understand that my decision regarding photography or video does not affect my ability to train at Reizen MMA.

10. RULES AND CONDUCT

I agree to conduct myself respectfully and safely while at Reizen MMA.

I understand that Reizen MMA may suspend or terminate participation for unsafe behavior, harassment, threatening conduct, intentional misconduct, repeated violation of gym rules or other conduct that Reizen MMA reasonably determines creates a safety or operational concern.

11. GOVERNING LAW AND SEVERABILITY

This agreement shall be governed by the laws of the State of Illinois.

If any provision of this agreement is determined to be invalid or unenforceable, that provision shall be limited or severed to the extent necessary, and the remaining provisions shall remain in effect to the fullest extent permitted by law.

12. ACKNOWLEDGMENT AND SIGNATURE

PLEASE READ CAREFULLY BEFORE SIGNING.

I acknowledge that I have read and understand this agreement.

I understand that martial arts and combat sports are inherently dangerous physical activities and that serious injury, permanent disability, or death can occur.

I understand that by signing this agreement I am voluntarily assuming certain risks and giving up certain legal rights, including rights to bring claims against the Released Parties to the extent described above and permitted by law.

I have had the opportunity to ask questions before signing.

I sign this agreement voluntarily.
